What counts as a capitalized word?

Any word that begins with an uppercase letter A–Z, which usually captures names, places, and other proper nouns.

Are duplicates removed?

By default only unique capitalized words are shown, in the order they first appear. You can toggle this off to keep every match.

Will it catch sentence-start words too?

Yes — words capitalized only because they start a sentence are also matched, since the tool can't know intent from capitalization alone.
